Form 945-X, Adjusted Return of Withheld Federal Income Tax or Claim for Refund is used to adjust federal tax deposits related to nonpayroll tax liabilities (e.g., backup withholding, pensions, annuities, and gambling winnings).
You must not use Form 945-X to claim a refund or request abatement. Instead use Form 843.
When Should You File Form 945-X?
There is no specific deadline for filing Form 945-X. You must file Form 945-X when you discover an administrative error on previously filed Form 945.
For Underreported Tax: If you have discovered underreported tax, you must file Form 945-X by the due date of the return for the return period in which you discovered the error and pay the amount you owe by the time you file.
For Overreported Tax: For overreported amounts, you may choose to either make an interest-free adjustment or file a claim for refund or abatement using Form 843.
If the due date falls on Saturday, Sunday, or a federal holiday, make sure to file Form 945-X on the following business day.

Check this box if you are correcting both underreported and overreported amounts. On checking this box, the negative number shown on Form 945 will be applied as credit.

Part 2: Enter the corrections

Line 3: Federal income tax withheld
To correct the federal income tax withheld, you must enter the total corrected amount in column 1 and enter the amount you originally reported or as previously corrected in column 2. Enter the difference in column 3.

Line 4: Backup withholding
If you are correcting the backup withholding of federal income tax you reported on Form 945, line 2. You must enter the total corrected amount on column 1 and enter the amount you originally reported or as previously corrected in column 2. Enter the difference in column 3.
